Trident Health and Trident Heart Failure Clinic are seeking a Physician Assistant or Nurse Practitioner to join their established team in Charleston, South Carolina.
- Active South Carolina state license, or the ability to get a South Carolina license prior to starting
- Great interpersonal and communication skills
- Demonstrates clinical experise, leadership, and teaching in patient care
- Chronic disease management working to establish plans for patients with CV Surgeons/Cardiologist
- Full Time Outpatient responsibilities
- Duties will include, but are not limited to follow-up care and commitment to development of a care plan for patients with chronic heart failure in efforts to prevent hospital readmissions and in accordance with state licensing board responsibilities
